Pet Health Certificate

Pet Health Certificate

When traveling with your pets, there may be animal health requirements specific to that destination. Dr. Jung is accredited by the USDA to provide Pet Health Certificate's for International & Domestic Travel.
As soon as you know your travel details, contact Noah’s Ark Animal Hospital to assist with the pet travel process.

Factors to consider may include meeting time frames for obtaining a health certificate, updating vaccinations, microchip, diagnostic testing, or administration of medications/treatments. You can learn more about those requirements at USDA/APHIS Pet Travel (www.aphis.usda.gov/aphis/pettravel)

Please note most international health certificates requiring USDA endorsement must be completed using the USDA’s website; Veterinary Export Health Certification System (VEHCS) and usually need to be accompanied with a prepaid shipping label. All forms need to be completed by a USDA Accredited Veterinarian.

Rabies titer test (OIE FAVN) samples collected here to send out for testing if required. Please call and ask about scheduling your pet's USDA Health Certificate and Examination.
